Windsor Family Homes & Community Partnerships may take photographs and/or video recordings during the Nourish & Nurture Program and related events.

By signing below, I grant permission for images and/or video of myself to be used by Windsor Family Homes & Community Partnerships for promotional and communication purposes. This may include use on social media, websites, newsletters, reports, and other marketing materials.

I understand that:

Images and videos may be used without further notice or compensation

All images will be used respectfully and in a manner that reflects the values of the organization

This consent is voluntary and may be withdrawn at any time in writing, understanding that materials already published may continue to be used.

I understand that participation in kitchen and food preparation activities involves potential risks, including but not limited to cuts, burns, slips, and allergic reactions.

By signing below, I acknowledge that I (and/or my child) am voluntarily participating in these activities and agree to follow all safety instructions provided by staff.

I hereby release and hold harmless Windsor Family Homes & Community Partnerships, its staff, and volunteers from any liability for injury or illness that may occur as a result of participation, except where caused by gross negligence.

I confirm that I have disclosed any relevant food allergies or medical conditions that may impact safe participation.
